2026-2027 Registration

I, the undersigned, hereby acknowledge and agree to the following terms and conditions set forth Freedom in exchange for my use of the facilities, equipment, and services provided by the program. By signing this document, I voluntarily assume all risks associated with my participation in activities at Freedom.

Assumption of Risk

I understand that physical exercise and training activities, including but not limited to weightlifting, cardiovascular exercises, group classes, and gym equipment, involve inherent risks and dangers. I acknowledge that these activities may result in physical injury, illness, or even death. I voluntarily and willingly assume all such risks and responsibilities for myself.

Medical Condition

I hereby affirm that I am in good health and have consulted a medical professional regarding my ability to engage in physical activities at The Freedom Institute. I am unaware of any medical condition, injury, or ailment that might affect my ability to participate safely in physical exercises. I will immediately inform the program's staff of any changes in my health condition that might impact my ability to engage in physical activities.

Release of Liability

In consideration of being permitted to use the facilities and services of the program, I hereby release, discharge, and hold harmless Freedom, its owners, directors, employees, agents, contractors, and all other individuals associated with the program from any and all claims, actions, damages, liabilities, costs, and expenses arising out of my participation in activities conducted at the freedom facility, including but not limited to personal injuries, property damage, or any other harm, whether caused by negligence, breach of contract, or otherwise.

Indemnification

I agree to indemnify and hold harmless Freedom, its owners, directors, employees, agents, and contractors against any claims, demands, or causes of action brought by third parties arising out of my participation in activities or my breach of any terms of this agreement.

Severability

If any provision of this Liability Waiver and Release Form is deemed to be invalid, illegal, or unenforceable, the remaining provisions shall remain in full force and effect.

I have carefully read and fully understand the contents of this Liability Waiver and Release Form. I am aware that by signing this document, I am giving up certain legal rights and remedies. I voluntarily and knowingly sign this document as my own free act and deed.

This Contract Agreement is made and entered into as of 8/10/2026 (the "Effective Date"), by and between Freedom and the Guardian of the student named above, (referred to as the "Client").

1. Services

The Institute agrees to provide educational services to the Client by the terms and conditions outlined in this Agreement. The services are outlined as homeschool support (collectively referred to as the "Services").

2. Payment Terms

The Client agrees to make the following payments:

a. The Client shall pay a non-refundable deposit of $500 upon signing this Agreement, which will be applied towards the total cost of the Services.

b. Ten (10) monthly payments of $1,200 each shall be made by the Client, starting in August and continuing for consecutive months until the full amount is paid. The total amount payable by the Client under this Agreement is $12,500.

3. Payment Schedule

The Client shall make each payment by the first day of each month, commencing on August 1, 2026 and continuing until the total amount is paid. The Client shall submit the payments to the Institute online or any other method agreed upon by both parties.

4. Late Payments

If the Client fails to pay within ten days of the due date, the Institute may charge a late fee of $50 for each late payment. The Institute reserves the right to suspend or terminate the provision of Services if the Client fails to make timely payments.

5. Non-Refundable Payments

All payments made by the Client under this Agreement are non-refundable, except as otherwise stated in this Agreement or as required by applicable law.

6. Termination

Either party may terminate this Agreement upon written notice to the other party. In the event of termination, the Client shall remain liable for any outstanding payments due up to the termination date.

7. Confidentiality

Both parties shall maintain the confidentiality of any confidential information exchanged during the provision of Services. This obligation shall survive the termination of this Agreement.

8. Governing Law and Jurisdiction

This Agreement shall be governed by and construed by the laws of the State of Florida. Any disputes arising under or in connection with this Agreement shall be subject to the exclusive jurisdiction of the courts located in Florida.

9. Entire Agreement

This Agreement constitutes the entire understanding and agreement between the Institute and the Client and supersedes any prior or contemporaneous agreements, deals, or representations, whether oral or written.

10. Amendments

No modification, amendment, or waiver of any provision of this Agreement shall be effective unless in writing and signed by both parties.

I, the undersigned, hereby grant permission to Freedom, its representatives, employees, or any third party acting under its authority, to use and publish photographs, video recordings, or other media containing images of the students listed below.

I understand and agree to the following terms:

1. Authorization: I authorize the Institute to use the photographs, video recordings, or other media in various forms of print and electronic media, including but not limited to the Institute's website, social media platforms, newsletters, brochures, promotional materials, and any other mediums for educational, promotional, advertising, or archival purposes.
2. Identification: I understand that the photographs, video recordings, or other media may include the names and likenesses of the students identified below. I grant permission for the Institute to use these names in connection with the images.
3. Release of Claims: I release the Institute, its representatives, employees, and any third parties acting under its authority from any claims, demands, or causes of action arising out of the use, alteration, or distribution of the photographs, video recordings, or other media, including any claims for defamation, invasion of privacy, or infringement of intellectual property rights.
4. No Compensation: I understand that the students listed below or I will not receive any compensation for the use of the photographs, video recordings, or other media.
5. Revocation of Consent: I understand that I may revoke this consent by providing written notice to the Institute. However, the revocation will not affect any uses of the images that occurred before the Institute received the written statement.

Behavior

1. When interacting with both students and teachers, it's essential to maintain a positive and polite demeanor in both speech and actions.
2. Participate actively in class discussions and activities and follow directions without hesitation.
3. Always strive to do your best and inspire others to do the same. Avoid any actions that may distract your classmates from their studies.

Academic

1. To ensure a successful class experience, being prepared is essential. Arrive early for the classes that you are attending (classes start at 8:00 AM, 9:15, 10:30 and 11:45 AM Monday through Thursday), have all the necessary materials, and organize your work ahead of time.
2. The student is responsible for completing follow-up activities from class.
3. Ensuring assignments are finished and submitted within the deadline is vital.

Plagiarism Policy

1. Submitting someone else's work as your own on an assignment is unacceptable.
2. As a helpful tool for students, we will incorporate various AI features. Nonetheless, it is essential to note that solely relying on AI and presenting it as one's work will not be deemed acceptable.
3. It is only allowed for students to utilize any technology during exams if specifically authorized by the instructor. Furthermore, they are prohibited from referencing other students' papers.
4. It is crucial to cite sources of information when submitting assignments.

Technology Policy

1. Our clients are treated with respect and autonomy at our establishment. They are permitted to possess technology such as cell phones, but it is expected that phones are set to silent during periods of learning. If a student must use their phone to communicate with another individual, they must do so outside the classroom setting.
2. Headphones and other electronics are not allowed during instruction. Students can have these items during open lab time if they want to use them while they work.
